Resources designed to help you shop, cook, and eat well
Our Cooking Up Change program at Near North Health was created to promote better health for all by empowering patients to take an active role in managing and improving their nutritional wellness.
Patients learn tools for a healthy lifestyle and disease prevention/management through outreach, workshops, individual counseling, and single-session or multi-session group classes.
By providing education, increasing awareness, and offering support and self-management tools, we’re making it easier for patients to practice healthy behaviors and manage their health and/or disease.
Clinical Nutrition: A key part of our Cooking Up Change program
NNH’s registered dietitian provides both individual and group counseling and follow-up to adults and children in need of nutritional therapy for the management of diabetes, heart disease, hypertension, hypercholesterolemia, HIV/AIDS, and more.
Our program includes services surrounding:
- Pre–Diabetes
- Type 1 & 2 High Blood Pressure/Hypertension
- High Cholesterol Overweight/Obesity Stress
- Coping/Management
- Exercise/Meditation
- Healthy Eating
Near North Care Coordinators will:
- Work with patients to develop individualized healthcare plans
- Work with providers to schedule appointments and obtain required referrals for needed
- Provide ongoing case management services
- Participate in systematic efforts to monitor the appropriateness of treatment
