Rates adjusted by income help us make sure Chicagoans who are uninsured, under-insured, or otherwise unable to afford standard healthcare fees always have access to the quality care they both need and deserve.

Sliding Scale Fees
NNH’s Sliding Scale Fees are based on the size of your family and your household’s annual income. If you qualify, you’ll pay a reduced rate for most of our health services – and you may qualify even if you already have health insurance!

How to Pay for Services
Near North Health accepts the following methods of payment for services:

Discount Category A: $30
Discount Category B: $40
Discount Category C: $50
Discount Category D: $60
Discount Category E: $70
*Discounts are determined by income and family size, following the current Federal Poverty Guidelines. The nominal fee covers office visits (new or established) and basic in-house lab tests, such as the Urinalysis Routine Test, A1C Hemoglobin Test, and Glucose Test. Additional lab tests and services are billed according to the Allowable Schedule.

La tarifa nominal cubre visitas médicas (nuevas o de seguimiento) y pruebas de laboratorio básicas realizadas en la clínica, como el análisis de orina rutinario, la prueba de hemoglobina A1C y la prueba de glucosa. Las pruebas de laboratorio adicionales y otros servicios se facturan según el horario permitido.
