Before Your Appointment

Collect all the items on the “What to Bring” list. If you are a current patient, plan to arrive 15-20 minutes before your appointment. If you are a new patient, plan to arrive AT LEAST 30 minutes before your appointment.

Cancel or Reschedule

If you are unable to attend your scheduled appointment, please let us know as soon as possible. You can:

After Hours Non-Emergency

A provider is available for after hours calls when your Near North Health center is closed. Call your health center or our main line (312-337-1073) and someone will assist you.

What to Bring to Your Appointment

Please have the following with you for your appointment.

Identification (at least one of the following):

  • Driver’s License
  • State ID
  • Employment ID
  • Public Aid ID
  • Utility Bill indicating name and address of patient, parent or guardian

Proof of Insurance

  • Medical or Insurance Card

Identification (at least one of the following):

  • Driver’s License

Verification of Income

  • Two recent check stubs or W2 Tax Statements.

Identification (at least one of the following):

  • Proof of Income (Weekly, Biweekly or Yearly)
  • SSI Benefits
  • Unemployment Benefits
  • Disability Benefits
  • Child Support Payment with copy of child support agreement

Is This Your First Visit?

If you’re a new patient, you can save time when you come to your appointment by completing our Patient Registration form before you come. If you have access to a print, please complete, print and bring your registration form with you to your appointment.
