Near North Health (doing business as Near North Health, or NNH) is committed to treating and using your protected health information responsibly. This Notice of Health Information Practices describes the personal health information that we collect and how/when we use or disclose this information.

Understanding Your Health Record/Information

Each time you visit one of NNH’s health centers, a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment and a plan for future care or management.

Get a Copy of Your Medical Records

If you are:

  • A patient over 18 years old
  • The parent or legal guardian of a patient under 18
  • A patient under 18 with legal rights to consent for yourself
  • The legal guardian of a patient over 18 with written patient consent
  • The legal guardian of a patient over 18 who does not have capacity to consent

… then you can request a copy of your medical records! Just fill out, sign, and send your Medical Record Release Form to:

Authorized Medical Records and Patient Record requests can be either mailed to Komed Holman or emailed to nnhmedrec@nearnorthhealth.org.
